You must pre-register with Dr. Liesen by email or phone, .

Payment will be individually per course, at the course, for those that have pre-registered.

Continental breakfast is included for all paid attendees.  Lunch will be available, but is not included in any tuition.

* T.L. Gilmer member Annual CE Dues:  T.L. Gilmer members have an annual CE dues rate of $250.  This allows them access to any or all CE courses for the term September to May.  They are responsible for their lunch at each course, unless they prefer to pay their CE Dues to include all lunches, which is then $310 for the year.  Any unused lunches are non-refundable.

Non-Members:  ADA Member tuition is $200 per course.  Non-ADA members tuition is $250 per course.  Either option includes breakfast and lunch.  You must have paid course tuition prior to registering for a course with Kara Manfredo at the ISDS if you want to receive CE credit.  If you pay the treasurer on-site, you will not be registered for CE credit with the Illinois State Dental Society.
